This is an interesting project:

I need to train this some more but I've got the left-clicking to work with the "hmm" sound pretty reliably. Response is extremely fast!

It looks like this can be used in combination with Dragon® (although Dragon says "please say that again" it's not performing any actions upon the "hmm" sound, not on my system anyway).
