Using Dragon Pro Individual 15.61 with an Andrea NC-181 USB headset on Windows 10 PC. Since upgrading to Dragon 15, I  find dragon intermittently but frequently misses phrases or sentences entirely, mainly in the middle of dictating a series of sentences, during which I am thinking and tend to pause at times. I tried Nuance who had me create a new profile and import my previous vocabulary, but have not seen any real improvment under the new profile. I have a second computer from work that has the same version of dragon (under a different license) and notice it has the same problem. It's very frustrating and any help is appreciated.

This could be an audio quality issue. Potential sources are almost endless: a microphone that is past its prime, a microphone that is not positioned properly, environmental sounds, and dictation habits, to name a few. An example of the latter is starting to dictate a phrase with a word like "select" or "correct" or "bold." So if someone says, "select one of these options," Dragon will attempt to select the phrase "one of these options." If not found, Dragon disregards the phrase.

Personal habits can lead to these kinds of problems. I've seen Dragon omit entire sentences for people who handle or hold the microphone boom as they dictate. Touching the boom can introduce noise into the audio stream.

When it happens next, the first thing to do is check your recognition history. What does Dragon report it has heard? Utterances marked with an "X" indicate commands.
