I come here every few months. The best mikes, new tricks, something to avoid.

 I would like to make using DNS faster in the hospital. I think the most appropriate question would be , is there a website that offers templates?

 We are  busy. The hospital appears to be laughing at us when we ask to integrate. So I have to dictate labs myself. And I really cant find  quiet place to work besides being right in front of the patient.

 Any words of wisdom that can help me speed up the process> we are still on paper. The vitals and meds are in front of the room and the chart usually ....... Somewhere.

If you were using Dragon Medical 10.1 you would have the advantage of being able to create time-saving commands, medical vocabulary, medical templates and even built-in commands that are designed to be utilized with medical templates. Using DNS Preferred is equivalent to putting bicycle tires on a sports car. Example: In NaturallySpeaking Medical you can automate a template by placing a pair of [ ] (brackets) after the patient name, date etc. To jump to the next field you simply say Next Field. There are also specialized handheld microphones with programmable buttons that will allow you to program microphone on/off, momentary microphone on, jump to the next field etc. and they work well in front of patients who don't like to see their doctors wearing a geeky looking headset. The Dictaphone PowerMic II USB microphone is so easy to program that all you have to do is type the name of the command that you want to deploy which can be anything; even a personal command. The new Philips SpeechMike Air wireless handheld USB microphone is the latest contender and the Philips SpeechMic Pro Plus is the traditional microphone most commonly used by physicians. Note that the PowerMic II only works with Dragon Medical and Small Practice Edition. Drat!

We apologize if this sounded like a lecture but we do not know what you are using for a sound system because you haven't completely filled out your forum profile but trying to work with DNS Preferred in a medical environment is equivalent to a professional mechanic purchasing knuckle busting tools at Kmart.

We recommend contacting a
speech recognition specialist who can help you decide on what the best microphone would be for your particular needs and additionally assist you with creating time-saving templates. If you had a heart problem, you probably wouldn't go to your local GP. You would no doubt seek the advice of a cardiologist. Speech recognition is the same way. If speech recognition is important to your workflow, we recommend contacting a Nuance-certified speech recognition specialist and we just happen to know of one

not enough information.  However, if you are still on paper, are you dictating to Word and then printing out your dictation to post on the chart?  An EMR can automatically generate information within your note.  Our Cerner Powerchart automatically posts the problem list, medication list, vital signs, I&O, lab results last 24 hours, pathology results last 48 hours, micro results last 2 days, and several other items to the daily note, all at the preference of the physician.  The physician types or dictates additional subjective and plan and whatever else they wish.  Other EMR's (Epic, Eclipsys, McKesson, etc) probably do the same.

Our hospital medical staff organized a physician technology committee seven years ago that began working with the MIS dept to develop a strategy for transitioning to an EMR.  Although the hospital pretty much did what it wanted 75% of the time, there was nevertheless a lot of input from the committee and some accountability to it.  Perhaps you need to organize such a group, especially with the government incentives and future penalties for not having an integrated EMR.  Google "meaningful use" to find the new government plan for promoting (and requiring) health information technology.

 You could create your own templates directly within Dragon.   I know of none online.  Only an interface could deliver results to your note, though.

I appreciate the responses. I have "created" my own EMR which allows easy communication with PMDs and CPOE if given the opportunity.

 I don't usually buy from Kmart, for some reason their stuff doesn't last. I also bike and so I would install Powertaps instead of regular bike tires, and my goal is a Porsche Cayman. My six kids would never fit though.

 My main interest in purchasing preferred was to be able to add Trigrams speech module and learn from there. OK for now, and I may eventually upgrade to Medical. I am the computer doc for the group and we are in relative straits, hence cheapness takes precedence until I feel the expense is appropriate and the demand is there.

 Given your stature as a DNS supplier, I would be happy to receive any hand me downs, unneeded CD ROMs containing DNS medical or Power Mics that are lying unused.

 My only issue with interfacing for us as hospitalists is human error. Missing a lab or investigation in haste and, in turn, putting the patients care at risk. I don't know if I am all for it.

We don't disagree with your “testing the waters” approach but the one thing we don't like about the Trigrams specialty vocabularies is that each one is contained within a blank user profile which means virtually starting over and tossing out all of your previous training. You are also limited to using one vocabulary. We took a different approach with the KnowBrainer Medical Language Models (which also works in DNS Preferred) by building an installer that imports one or more vocabularies into your existing trained user profile. This type of installation gives you the advantage of preserving your existing training/pronunciations and the option to import more than one vocabulary.

NaturallySpeaking, Windows and Microsoft Office now utilize DVDs but there is no such thing as an unneeded CD or DVD because these are only media storage devices for the application. In other words you can make as many copies of NaturallySpeaking as you want but you still have the issue of the serial number which has a limited amount of activations and is licensed, by the manufacturer, to you alone. However, there is always the chance that someone could retire their serial number by unregistering and deactivating NaturallySpeaking and passing the information to you or even donating one of those new $445 MAP priced PowerMic IIs but we suspect the likelihood of that happening would be about equivalent to winning a $1 million lottery. HOWEVER, having said that, we occasionally receive deactivated copies of NaturallySpeaking (most commonly DNS Standard and Preferred) and slightly used microphones which we usually donate to the physically disabled or people who couldn't otherwise afford to purchase DNS or an adequate microphone. 

There are a number of reasons why the street price on DNS Preferred is only about $170... but perhaps the best recommendation we could make for the professional versions of NaturallySpeaking (Small Practice, Medical, Legal and Pro) is that #1 we use this technology to run every aspect of our company and #2 NaturallySpeaking includes a manufacturer backed no risk customer satisfaction no restock fee 30 day guarantee.
