So, some of you might of known that I was having computer issues with my old xp 4g computer and after 3-4 months of changing hard drives, killing viruses, screwing with registry errors I finally bought a new computer (yes , last week right before the new boards , processors come out, shoot me now!!). 

After my computer died and I still needed to do like 25 dictations (sometimes I just dictate all at the end of the day and sometimes through out), I have had enough and had to get the computer up and running quickly for my wife (the other doc in the practice) since when your wife is happy.....well you know the rest.....

I have a Tiger Direct close to my office and read the reviews on the Systemax computers that they have. They were all great so I decided to get the SG-125 Intel P67 I7 GTX 550 Gaming pc. These systems do not have bloatware, come with 8g (I matched the same crucial class 10 memory with another 8 for like $38.00,pc3-12800 memory with heatsink). These come with Windows Pro 7 64 bit and I have MS office 10 plus at home. These are nicely built and use decent wire, msi p67a-c43 atx motherboard, 1 tb drive 7200 sata 6g with Nvidia Geforce gtx550-ti 1gb video card, blue ray drive, decent Vortex 3620 gaming case and a 550w 80 plus bronze power supply. Yes, I would like a 700 power supply but so far so good. Super quiet also.

 I like to thank Thanos over at Textalk Enterprise Electronic Medical Records for downloading and getting me setup over the weekend........Read the rest to see how Textalk speech emr works with DPME. 

 So I did all the windows and ms updates. I use ESET 32 version 5 antivirus and adobe 9.5 not 10(x). Use an online scheduler call Eppointments plus, have Log Me in Central. 

I have had the Speechware 3/1 and the new board sent to me but did not use it since having so much issues with my computer. I did use it at home a little. I did "actual" surgery to remove the resistor and put in the new board. I did use it with this system prior to doing this but it needed to be done.

So the question is "how fast is fast"?????

I want the new boards and processor coming out soon etc but this was $999 on sale and all I can say is this is a very fast computer. Thanos even made a comment on how fast things were loading. 

I trained a brand new user using the speechware 3/1. I wanted everything to be fresh. I still have my old user if I want it. I am using Best Match 3 medical not 4  and using Podiatry module. I have added a few words that I know are not in the module that I need. I have not used KB11 yet. I just want this to be DPME for this testing. 

 I put the accuracy level to 100% since when I started some dictation I had it on 0 or all the way to the left and there is a "noticeable" difference on a new user. When I put it on 100% accuracy since I did not need the speed it was magic for me. 

I did a few other tweaks but all mentioned in this forum.

Remember also I am getting used to using a table microphone since I have been using the Senn Me3 now for like 4 years or so. I had to find the sweet spot for me to use the Speechware 3/1 mic. I found that I was leaning into the mic more then I should. I measured and I tend to be about 14 inches to 16 inches away. I am practicing being more like 18 to 20 inches away. This is just me getting used to using a table mic .

Prior to using this computer I had a delay with Textalk Speech EMR with DPME that I did not have with version 9.1 dragon medical. When I would say "Choose Patient" (record number like 120535) this would bring up the patient case but had a few second delay with DPME on the xp machine. Then I would say "Progress note form" and the progress note with automatically fill in the date, patient name, referring doc, medical record number and put the cursor at my starting point,  etc etc etc but again a delay with the xp machine with DPME that I did not see with my version 9.1. 

When I say I have no delay now at all I am aware that there has to be an actual delay. I will use my RVP1 program that I use with my phone to capture my son's baseball swing and can replay it in slow motion, one day to see the "actual" delay while dictating but unless I dictate a full paragraph in one breath, it is hard to see a delay at least. 

All my built in EMR commands work perfectly, in fact all my macro's etc work perfectly. Did I mention how fast everything was??????

Accuracy: Well before setting the setting to most accurate, it was just ok but once I made the change to 100% accuracy (all the way to the right), that was a noticeable change and improvement for me. 

I rarely correct mistakes in the last week of use. I do all my referral letters to docs at one time, all my progress notes at once and all my new H&P's at once or at least it seems that way on most days. I like getting into a rhythm per se. I have more "misspeaks" then errors. I do have to use better enunciation since I dictate at a very fast rate. I call it hyper-speed.......  When I slow down to human speed there are simply no errors. I would never say 100% accurate but this has to be close. I do notice that if I say a word and it comes up wrong then the first choice is the correct word so I am just getting this user used to me. If I say "spell that" or "correct that" there is simply no delay per se when correcting errors. I have no issues that were reported prior with the backspacing etc. Things just seem to work now. I have to admit that I thought DPME was quirky on the xp machine but evidently it was issues with the hard drive and lack of processing power more then DPME issues. 

I do think that in DPME that there are language model issues as proven by some when they changed from one module to another one and solved their issues.

Yes, I wish there were updates and all that but coming from 9.1 I see a big difference now with this new computer and mic system. I have used the Senn ME3 as a piggy back on the Speech ware 3/1 and it is really outstanding with this glorified sound card!!! 

Now I have played around with version 10 medical. If I had 10 medical and was happy with the performance, I would personally stay with that version for now. If I was a new user with a fast computer, I would go to DPME.

DPME on a slow computer, even my wife's Toshiba with 4g memory seemed quirky. If you have been using Dragon long enough you will know what I mean, hard to describe but not smooth like butter. DPME on a fast computer, as least for me, is smooth like butter and not quirky.

So a fast computer with a great microphone and sound card equals a happy doctor.
