I manage multi-user environment and have encountered a strange problem with v9.1 (SP1) Medical.  A user-defined command within a command lists suddenly stop working.  The command is in the form of

<shortcuts>

shortcuts is a list of commands the user can isssue that in turn executes a Microsoft Word macro
The <shortcuts> Dragon script is in the form of

Sub Main
  Word.Application.Run "mymacro", ListVar1
EndSub

The strange thing is that this script worked just fine when we started using it.  But now, some commands within <shortcuts> stop executing.  The rest of the commands execute just fine.  Dragon seems to recognize the command because it types it out in Word as regular dictation.  Forcing command mode didn't work either.  When I renamed the command, it works again.  This is a work-around but then it requires my users to learn/remember new commands.  This problem happens on more than one users on different PCs so it is not hardware dependent.

Has anybody encounter similar problems?  I am stimied, any pointers as to how I can solve this problem is very much appreciated.

In order for your macros to work, NaturallySpeaking has to completely get along with Microsoft Word.  Although NaturallySpeaking works well with Microsoft Word most of the time, it simply isn't foolproof. Even if you were able to solve a specific problem, it would probably reoccur. The smart money might be to remove as much complexity as possible and that means rewriting your Microsoft Word commands into NaturallySpeaking Medical 9.  The built in Command Browser is one of the reasons why Medical 9 costs as much as it does and custom macro deployment is one of Medical 9's principle features. Your end-users won't have to learn any new command names because from their point of view, they will work the same way but you'll have the extra work of porting the commands over for Microsoft Word to NaturallySpeaking.
