Rheumdoc, Instead of setting up your template choices as you have, why not use this approach (see below) inserting variables. You can navigate between variables via voice by saying: Next, then fill in the default value saying Yes, No, Unknown etc. You can do this in the Command Browser via a Text and Graphics command, using the Variable radial button, bottom right. You can even paste your current templates here, highlighting each area you wish to turn into a variable one by one, then clicking Variable. History of smoking: [default value] History of alcohol: [default value] History of chronic use of corticosteroids: [default value]

Rheumdoc, I should have also mentioned that your microphone, Plantronics CS50 bluetooth wireless USB, is not conducive to great accuracy using Speech Recognition. You will get more mistakes with this microphone. Given that you are using it for work, it is worth the investment to get a good microphone remembering that time is money, and time is time! It will more than repay your investment, and lessen your frustration. See: http://www.knowbrainer.com/NewStore/pc/viewCategories.asp?idCategory=15 MDH

1. The most likely reason why you're experiencing a delay on your office computer is because you are suffering from some form of interference. Try rerunning the Audio Setup Wizard and pressing the Play button in the last stage. We suspect that you will hear something such as a 60 Hz hum which is the most common form of interference but you may also have an issue with your CS50-USB microphone. The CS50-USB simply doesn't meet our minimum requirements. Note that if you are suffering from interference or just a substandard sound system, NaturallySpeaking will not only be less accurate but it will additionally struggle to decipher your dictation which can result in the delay you are experiencing. You might think of NaturallySpeaking as a sports car and your current sound system being the equivalent of bicycle tires.
2. MDH’s recommendation would've been right on the money for NaturallySpeaking Pro or Dragon Medical but this is a one of the caveats to using the Small Practice Edition. The next field command simply isn't available in the Dragon Small Practice Edition but is available in KnowBrainer 2011 as Fill In. Speech recognition can only be as strong as your weakest link. Dragon Medical Small Practice is a very powerful application but the included OEM microphone shouldn't be considered as more than a starter microphone. Consumer grade microphones such as the CS50-USB and even the microphone you bought from a local retail store are typically only marginally better. If speech recognition plays an important role in your workflow, we strongly recommend purchasing a professional soundcard & microphone that is equivalent to the quality of your speech recognition application. -------------------------
